Becoming a member of our concierge practice is an investment in your health and well-being. If you are interested in becoming our patient, please call our office at (301) 545-1811 for more information and to request a complimentary meeting with Dr. Seidman.
During this one-on-one consultation, you will meet with Dr. Seidman for 30 minutes to get acquainted. You will also have the opportunity to tour the practice and meet our incredible staff.
The doctor will ask what you are looking for in a primary care physician and what your experiences have been like in the past – both positive and negative. We want to know what your major needs and concerns are to ensure those needs can be properly addressed. The doctor will also discuss the philosophy of care at Rockville Concierge Doctors so you know what to expect.
Based on all of this, you and the doctor can determine if Rockville Concierge Doctors will be a good fit for you. If you decide to become a member, you can complete the forms below to get started.
Annual Membership Fees and Payment
- For patients 18-39 years old, the yearly membership fee is $1,724.00.
- For patients 40-67 years old, the yearly membership fee is $2,372.00.
- For patients 68-89 years old, the yearly membership fee is $2,542.00.
- For patients 90 years old and older, the yearly membership fee is $3,144.00.
The annual membership fee entitles you to unlimited office visits, consultations by phone or patient portal when possible, 24/7 availability to a physician in the practice, and certain basic blood tests with your physical. There are no additional co-pays or fees.
Rather than the 15-30 minutes that most insurance-based practices allot per visit, we spend anywhere from 30 minutes to 2 hours with our patients.
If you become a member of our practice, you still need to maintain health insurance for such things as lab tests, imaging studies, referrals to specialists, and hospitalization. Rather than bill your insurance for each visit, we provide you with a receipt for the value of care provided, which you can then submit to your insurance company for possible partial out-of-network reimbursement. Please check with your insurance company for more information.
Discounts: If you are 40 years old or older, and a family member joins who is 40 or older, we will discount the less expensive fee by 10%. This discount continues each year.
Payment: You can pay your yearly membership fee by check, Visa, MasterCard, Discover Card and AMEX, either once or twice a year. If you have a Health Savings Account (HSA) or Flexible Spending Account (FSA), check with your HR representative or HSA/FSA plan manager to find out if your plan will reimburse all or part of the membership fee.
